Services Referral System

As part of Kent State Stark's Master Advising Plan (MAP), the following online referral forms are to be used to direct students to a variety of services to assist in academic success and persistance. 

Please complete the form with the student and print a copy for the student's records and reference. Following up on this referral and contacting the appropriate services is the responsibility of the student.

Once the form is submitted, it will be sent directly to the main contact for each of the service areas.
